Information Required Prior to Assessment
TCCS and DUI School has offered substance abuse and anger assessments to the Chattanooga, TN area since 2003. The following information is required at time of assessment:
- Photo ID
- 12-month history and proof of prescription medications
- Legal history including arrests, convictions, and probations
- Mental health diagnostics and treatments
- Substance abuse history and treatments
- All medical diagnostics, treatments, and surgeries
- Family history
- All treatment completion or course certificates

Additional Details About Our Assessments
- Assessments are scheduled by phone only
- Assessments take about one hour
- Allow two weeks to receive the results
- Bring any referral information if your results need to be sent to another entity
Substance Abuse & Anger Management Assessments
- Substance abuse in TN and GA requires a basic assessment – $175
- Substance abuse out of state (for states requiring a basic evaluation) – $175
- Substance abuse out-of-state packet
- $400 for NC
- $600 for SC, IL, NJ, AZ, MI, HI
- Anger Management Assessment – assessing if the client would benefit from anger management classes or Batterers’ Intervention Program – $115
If your DUI was in the states listed above, you must bring all paperwork and the packet that was mailed to you by your offending state.
Once you schedule an assessment, there are no refunds. A deposit is required during scheduling. If you do not appear for your appointment, you forfeit your deposit and you must pay again to schedule.
